Insurance Submissions

Cold Creek Counseling can submit claims to the following insurance payers:

 

Medical Mutual of Ohio

Aetna

Anthem Blue Cross Blue Shield

 

Submission of claims does not guarantee coverage. Cold Creek Counseling does not verify insurance coverage and clients are ultimately responsible for payment. Please contact your insurance company to verify coverage before your first appointment. If you are scheduled for a Telehealth appointment, please verify if your insurance company covers that type of service delivery.

We are always looking to expand on our insurance provider affiliations. Please email us if you do not see your insurance company listed.

If you have insurance that we are not affiliated with you may still be able to seek out of network reimbursement. We can provide documentation of session that you then send in to the insurance company for them to pay you directly.

Private Pay

 For private pay Cold Creek Counseling offers an income based sliding scale to help make services as affordable as possible. Click HERE for sliding scale income brackets and private pay prices. Private pay price per session will be the same for office based or telehealth services.
